This improved version of the popular McGee Pistons features a platinum ribbon loop attached to a stainless steel piston. Platinum offers good tissue tolerance and is eassier to bend into shape. In its flattened form, it permits a greater area of surface contact with the incus, lessenging the opportunity for pressure necrosis. Each piston also includes a 0.5 mm depth groove to assist in visualization of footplate penetration. This groove is clearly visible at virtually aby angle. A choice of piston diameters is available. |

A clinically proven piston design in a choice of well-tolerated material - titanium. The opening in the "bucket" of the prosthesis is modified to allow the incus to assume its corrected anatomical position minimizing the danger of pressure necrosis. The bucket handle fits easily over the lenticular process eliminating any need for crimping and the postoperative chances of presure necrosis. The Bucket Handle Prostheses fits either the left or right ear. A hole in the posterior bucket allows for pick insertion to aid in positioning. Two transaxial holes in the distal end of the prosthesis stern allow for tissue ingrowth to aid in the stabilization of the implant. |

Reconstructing the sound-conducting of the middle ear with this precisely engineered fluoroplastic piston closely approximates the natural configuration of the ossicular chain with stable implant attachment in both the vertical and horizontal planes. The unique "cupped" head of the Shea Fluoroplastic Cup Piston accepts the lenticular process of the incus, directing the piston shaft downward at the proper angle to the oval window. A special retaining loop behind the "cup" fits easily around the lower end of the incus to maintain the prosthesis securely in place. |

Reconstructing the sound-conducting mechanism of the middle ear with this precisely-engineered piston closely approximates the natural configuration of the ossicular chain with stable implant attachment in both the vertical and the horizontal planes. The cut-out "cupped" head accepts the lenticular process of the incus, directing the piston shaft downward at the propper angle to the oval window. The platinum arms are preformed to easily slip over the incus, and may be locked in place using only one hand with closing forceps. Fits either left or right ear. |

A clinically proven piston design in a choice of well-tolerated material - fluoroplastic. The opening of the "bucket" of the prosthesis is modified to allow the incus to assume its correct anatomical position minimizing the danger of pressure necrosis. Restablishes the ossicular chain when the incudostapedial joint is missing. Eliminates the need for a bone chip interposition or removal of the incus. |
| Recommended Surgical Technique*: |
| 1. |
Elevate a tympanomeatal flap and established good exposure of the incudostapedial joint area. Palpate the incus and stapes to ensure their mobility. |
| 2. |
Determine the appropiate size of the implant required by aproximating the size of the defect. The small prosthesis is generally suficient for most defects with the large prosthesis needed for bigger defects. |
| 3. |
Grasp the implant with alligator forceps and slide the through portion under the long process of the incus. Place the "hole" over the head of the stapes. |
| 4. |
Visulize the head of the stapes through the hole that runs through the prosthesis in order to assure proper positioning of the implant. |
| 5. |
The prosthesis is self-supporting if it is positioned properly. Palpate the malleus to verify good motion transmission the prosthesis to the stapes. |

The Applebaum Incus Replacement System has been designed to closely resemble a carved homograft/autograft incus interposition implant. The Applebaum I.R.S. is made of dense hydroxylapatite ans is offered in 5 sizes, eliminating any need to trim. The implants are partialy cannulated to fit on the stapes capitulum, and the carved "notch" engages the malleus handle. This Prosthesis is designed for use in case of a missing and stapes superstructure but a mobile footplate. Each implant is formed of dense hydroxylapatite, a highly biocompatible material with a chemical composicion nearly identical to bone. This material may rest directly against the tympanic membrane with minimal risk of extrusion. |
To Use: Measure the distance from the footplate to the undersurface of the tympanic membrane and the offset between the centirline of the footplate and the handle of the malleus. Use the measurements to trim the prosthesis to the proper dimensions using a diamond burr and irrigation. From a pocket between the handle of the malleus and the tympanic membrane, place the top of the prosthesis in this pocket, and align the shaft into the footplate. |

Made of Plasti-PoreTM material, a porous high-density polyethylene which is well-tolerates in the body as an implant, the post of the prosthesis seats firmly on the stapes and replaces the malleus and the incus. The implant may be trimmed as desired. |
